Adding a 2nd lookup to an form in the new process UI will cause that form to fail to load every time.
For example, Account form has Primary Contact as a standard lookup. Now add a 2nd lookup to Invoicing Contact & place on the form. Save & Publish and soon as you open a record you get an error message and worse still you now cannot save any changes to the record at all. Only solution I've found so far is to remove the 2nd lookup - all then works fine again.

I tested this with a CRM Online trial organization (created with the Demo Builder app) and wasn't able to reproduce the error. I added a "secondary contact" lookup on both the account and opportunity forms and they seem to work just fine on the new process UI, at least on IE10.

Im in the same situation as Jukka, havent been able to reproduce the issue.
Is this an on premise deployment can you send us an extract form the platform traces?, do you get any error message?
Regards
Thanks for quick replies. Its crm online not on premise and had happened with 3 separate sites I've tried so far.
I can get the information that the error msg says it will send to Microsoft and send you that if any help but obviously no access to server logs.
Note that it only errors on the Process ui forms, classic view and info forms are fine.
What address can I email to?
Thanks
If you are able to reproduce this issue in a new CRM Online Org without any customization in place then Please open a support case with repro steps and we will look into this.
Thanks,
