If a Project Manager files an expense report for one of their own projects, the expense report line item bypasses approvals. Is there a way to make these route to the supervisor of the PM? If not, I'm thinking of writing a database trigger to do it.

I tested your scenario in SL 2011 SP1 and the project manager will see his own line items appear for approval in BP. Are you sure the Mgr T&E Review option is set to Required on the Additional Info tab in Project Maintenance for that project?

Is there a way to redirect the approval to the PM's manager when the PM submitted the expense report?
There isn't a way to redirect the approval just for this purpose. You can specify delegates when the project manager is out of the office and so on. The project executive could approve the project manager's expenses however someone would need to tell the project executive that there are line items for him to approve. The project executive wouldn't get any notices. But the line items would continue to appear in the project managers approval center.
