I've currently got three instances showing under my Dynamics admin console:
CRM (default) (Upgrade) - Instance Type: Default
CRM - Instance Type: Production
CRM Old - Instance Type: Sandbox
The top one, with an instance type of default, I don't think I created. There's no option for me to get rid of it. Does anybody know what it is?
Currently when you hit the "Dynamics 365" icon from the office portal, it brings you into the default instance. The only way to access my production instance is via the direct link.

The CRM (default) (Upgrade) - Instance Type: Default is more related with CDS. If you want to access other Dynamics instance from office
You could go to office Admin Center and login D365 Admin like following screenshot to view your instance list:
Or you could add the D365 Instance Picker Admin in tags as well: https://port.crm.dynamics.com/G/Instances/InstancePicker.aspx?
