I have setup a custom contact and account entity and linked them to the default appointments entity using a linked entity HCP Contact Account.
I wanted to know how:
1 - to setup the Required field in Appointments so that only the contacts in the respective Account are displayed. As shown below when searching on Clinic name ( ie Account) no data appears
2 - When the respective Contacts appear, how to multi-select the contacts
Business Usage: Sales Rep will visit Clinics and should be able to add GP contacts in the Required Field who attended the meeting. To provide a good user experience, we want to make it easy for the Sale rep to view only the contacts in the clinic and then select the GP contact attending the meeting.

Regarding your above questions:
1. A filter would be best here. Go into the field properties (Contact in this case), then create a filter (view) which will only return the results of contacts in the previously specified Account. Just remember, this filter will only work once the Account has been selected. This article should help you understand this: www.marksgroup.net/.../
2. The reason multi-selecting isn't working here, is due to the fact that the field you are trying to fill, is only expecting 1 value/record (one 'HCP Contact Account' in your screenshot above). Your best approach here is to create a sub-grid, with a relationship allowing any contacts listed, to be related to the account/whatever entity you want.
