After an update on 5th October 2018, Add Phone call activity is showing a subject field as mandatory. Previously, there was only a description field. Users are now having the following issues:
any help on this will be greatly appreciated
For the first question ,there's actually a bug issue in V9.0 or V9.1 which is confirmed by MS DEV Team that the phone call subject lost in Web Client. It would be fixed in the future version.
For the other questions, pls share more details like the above screenshot to let us help.

With regards to the second question, just to clarify that when you create a phone call activity in Unified Interface, it saves it as Open and you can amend the details such as call back date and to create a follow up activity later on when required, however, in Web Interface, when Phone call is added through Add Phone Call activity from Social Pane, it saves the activity as Completed no matter if the cal back details are added.
Also Create a followup activity option is not showing on the quick view form. This option should be set as a default on the form with the call back activity. The option for create a follow up activity is available on Main Phone Call form but it should be add in quick view form as well.

Clear. The following screenshot is my Demo operation in my Instance.
When I open the Entity record in Unified Interface, the status is the same with Web Client which is completed and Read only.
As you mentioned to 'amend the details such as call back date and to create a follow up activity later', I recommend that you could add a new phone call activity and set the date and Incoming Direction like following screenshot.
For the third question , did you mean the quick create form customization of phone call activity as follow?
Or the Phone call card form customization? I think both of them are available.

When you create a Phone activity in UI, it saves as Open but when you create a Phone a activity in web interface from the social pane Add Phone Activity then it save the activity as completed. There might be a possibility that UI and Web interface are using 2 different forms to create the phone activity?
The form for Quick Add Phone Call cannot be customised, please see below the screenshot. I believe Dynamics 365 is using Quick Form to create ADD Phone call from the social pane. Is this correct or is it another form?
It's strange. As I checked that Web client uses 'Quick Form' and UFI uses 'Phone call quick create form'.
The logic which create completed Phone Activity is defined from Quick Form.
I need to correct what I said before as this is not +New command function which could not be override by API.
From the following search I think currently there's no solution to modify it. I am so sorry.

We are not able to see the description at all for Phone Activities in the social panel. If we populate both Subject and Description fields - we see the Subject but we never see the Description. This happens if we create the activity from the social pane, the associated activities view or the quick create form. How were you able to see the description of the phone call activity in the social pane (or can you still see it) ?
On Phone call activity, it is showing Entity Name and the Subject however on Tasks activity, it is showing the Subject and the description.
We are also seeing this on our instance, strange i'm sure it was working previously.
Oddly this only seems to affect phone-calls, tasks display the description correctly.
We are experiencing the same issue.
Subject is not appearing in the activity pane unless the Subject AND Description is filled out. This just changed recently.
Any ideas on if this is temporary or not?
Microsoft has released a new update recently and now you can see both Subject and Description for Phone call activity.
This issue is still happening in my org.
