Sometimes I can't see the actual change. There is just an odd icon. What does it mean? Can I see the change somewhere?
the icon appear because the audit is stopped and after that started the audit again.
This usually means that the Auditing feature was disabled or someone removed the auditing data.

You can see the icon when audit record might be deleted by some user or may be auditing has disabled . So try to enable auditing and see the results for new record .
