I've created 3 new forms on the Quote entity.
For each of these the Quote ID nor the Revision ID will auto update.
If I go back to the original OOB Quote form, these auto update as expected.
Have I severed a link between these somewhere in creating the new forms? With these fields being mandatory it's causing havoc.

This will help us understand your problem better.
I cannot upload a photo as image uploading sites are blocked by my company.
However they would not show anything different to what I described.
Quote ID and reference ID auto-populate in the OOT Quote form, but do not with a custom Quote form.
I tested it in my environment and didn't have your problem.
Here are my test steps:
Create a new quote form. I have not changed the layout of the new form.
Create a new quote with the new form. The Quote ID and Revision ID are automatically populated.
Could you provide more information? How did you customize the new form?
I re-arranged the tab and section layout.
I removed or set to non-mandatory any field that was not required by my new forms, and added the mandatory fields I couldn't remove to a tab and section at the bottom.
I'd unset "read only".
I put read only back on and it works as expected.
