I need to count how many appointments each user have for each day.
Do you have any suggestion?
I need to use this number in a workflow in order to prevent the user to save the record if the number of appointments exceeds number 3.

I believe you can develop a custom workflow activity that will calculate the number of custom workflow activities for a user and use it in workflow.
As suggested above you could use custom activity workflow to get count appointments of the day.

You can develop a custom workflow to count the number of appointments of the user having for that day. Once you get the count to add the if statement whether appointment count is greater than 3. If it is greater than 3 then stop the workflow(it will prevent the record from saving).
