I customize a account page on crm 2011 and add 2 field on this page
but i don't know what's happened after save this page account and contact Page not opened and show error

Undo your form customization, save and publish CRM form. Check if form is opening or not.
if yes, add fields back to form and then save and publish and try opening form.
Firstly, don't panic
I would remove the two fields from the CRM form, save and publish.
This will hopefully fix the problem.
Then try adding one of the new fields, saving and publishing to see if that works and then add the second one.

I'm having the same issue but on the Task form. Unfortunately I can't access the form to remove the field I added, I get the "try action again". I can't delete the Main form nor can I create a new one. Each time I get the "try action again" error. What would you recommend?
This sounds tricky because you can't open the form to correct it.
in this case I would try and create a simple unmanaged solution from a CRM environment where the task form works or is standard and then try and import this solution into the CRM organisation with the dodgy task form and hope it overwrites it with one that is ok.
I created a service request with Microsoft and that was the first thing they tried except it didn't work. They even exported my Task solution to test on their end, they can see the field that I added and the form works for them. There's some kind of bug in our instance that they haven't been able to correct.
hmm I'm not sure really what to suggest
I would recommend working through this with the Microsoft support team as they will no doubt have more experience of this situation than me.
but you may need to do restore from a database backup but then you will have to worry about the data in your system and export/import data from after the last DB backup. but I would see what the MIcrosoft support team suggest
I've had the service request open for a week with Microsoft support and they haven't been able to correct the problem so I'm researching on my own. Thanks for your input.

My team and I encountered the same problem on the Marketing List entity on CRM 365 online. So this issue can happen on any entity and has been around for quite a while.
Here's how we solved it :
Wanna know the origin? Customizing forms using Chrome. We decided to stick to Microsoft's products and use Internet Explorer.

I did similar thing. Exported from my other organization new task form and then imported it to bugged entity, deleted old form and for now I'm not having any problems.
When you say "customize from Internet Explorer and stick to Microsoft's products" - I tried Edge and my jscript functions weren't working. In Internet Explorer, when customizing forms, I can only double click on the field and set it to form or tab then use keyboard arrows to place that field. I can't use drag and drop option as in Chrome.

I have this same issue, unable to open forms on a custom entity and unable to create new. Just wondered if you had an example of these tags that need to be removed? Also, when you say export the buggy forms in a solution will that not also add fields and the entity in the solution meaning that when reimporting you get duplicate entity and fields?
