The address entity allows us to store multiple addresses for an account. What is the recommended way of adding this to the order form so that the user can select a ship to address from the stored account addresses?
In CRM account entity itself allows to create two addresses. If you are using one address as Main address and another address as Shipping address then you can use CRM Mapping to populate address from Account to Order.
Either you can add button on order form and when account is selected for order, then on button click you can pull account addresses and show to user and based on user selection update order shipping address.

CRM 2015 has something called "Lookup address" on Order where you can select the BillTo/ShipTo address from the Customer (Account) addresses. I don't recollect having seen it in CRM 2013 but you can check.
There is another way people achieved this by using a custom entity for address but I wouldn't prefer using custom address for entity as you would loose lot of in-built functionality.
If a CRM upgrade is not planned in the near future and the requirement must be immediately addressed, then you could build a page similar to the one in CRM 2015 so that you don't have to re-train the users when you eventually upgrade.
I am encountering an issue related to the "Look Up Address" on an order after transitioning to the Unified Interface.
The window opens and does allow me to choose an existing address as a Bill to and/or Ship to address.
We encounter an error when trying to add a new address from the look up window. It opens the window to add the address, but when we click Save, it throws an error. Has anyone encountered this?
