I have created a button inside the Application ribbon, it's working fine for all entities expect one entity for some reason the button is not appearing, anything might affect the application button?
Check if there's a syntax error in the javascrpit associated to the form's ribbon of your entity. that may be the case for you, i have had the same problem once.

Try to check following
- Browser console to check any JS error .
- Any rules are attach to the button
- Check with different user /browser .
- Clear browser cache , cookie.
- Finally try to write below line in the form onload -
