How to create a simple check box in a form?
You could do it by creating two option field.
Please refer below url for reference:
1. Create a field with Type "Two Options".
2. Add it to the entity form
3. Double click the field on the form
4. In the Field properties window, click the Format
5. Make sure to choose CheckBox as the Field display
