I opened the create case form in new tab, When I try to refresh the form it is giving the following error in unified Inteface of dynamics.
And all the fields which are pre-populated are also remains as balnk.
Can anyone help me with this?

Can you share us more details about the steps you done through screenshot . It might be more helpful.
Initially I opened the create case form(in new tab) through customer search in UCI. The form contains some automatic populated fields.
Now if I try to refresh the page, it is giving that error.

Are you using Dynamics 365 Retail?

Could you tell me which app you are using?
According to your screenshot, it doesn't seem to be CRM.
You can see the name of the app at the top left of the unified interface.
It doesn't look like you are using CRM ? Which App you are using?
Is it Dynamics 365 Retails?
