No can't, you can have only maximum of 4 column.
No way. By OOB only limited options are available. Not even unsupported way to do what you want.
But you can customize with HTML web resource to have your own layout & embed in form. Then you have to take care of validations , save events, etc
Stay up to date on forum activity by subscribing. You can also customize your in-app and email Notification settings across all subscriptions.
