I'm using Dynamics 365 Online 9.x and installed Unified Service Desk 4.0 with sample data into it.
Can anyone please tell me where this file is located? Thanks.

Following steps worked for me:-
1. Close your USD.
2. Delete all the folders which exists at C:\Users\<loginUser>\AppData\Local\Microsoft\UnifiedServiceDesk\
3. Restart your USD & it will bring the complete information again.

Do you really need Notes Manager? It has some limitations. I would rather prefer to deactivate the OOB notesmanager and create your own notes manager if required.
If you want to use it as it is, then there should be a customization file got created in your configurations. ensure you have this customization file .
