When I create a new Form Field, I do not see an option for "lookup" fields. Is there a way to add a lookup field to the form? For example, we want to add "Source Campaign" - default the value - and hide it.

In CRM D365 to add the lookup field in the form Go to Customization->Open the Marketing entity->Add new field.
In the Data type select “Lookup” option and Target Record Type select the entity to whom you want to create the lookup like “Campaign response”.. etc
After entering details save the new field and then go to form page to add this attribute. Your lookup field in the form now select the default value and hide it.

Lookup fields aren't supported on forms today. I suggest that you enter this as a feature request on the Dynamics 365 for Marketing Ideas portal as community support helps shape our roadmap. And our PMs regularly review them.
