    You probably have a plan where you defined also beneficiaries - the plans shown in the screenshot have only dependents (I assume). also, you must scroll and see if there are beneficiaries and dependents defined. the rule is the following: if the plan has beneficiaries (in the setup), you must have beneficiaries designated in the personal contacts and they must have a percentage assigned to them, in case something happen to the employee.

    1. First check if you have setup plans with beneficiaries

    2. check if you have beneficiaries for this employee

    3. check if the % for each beneficiary sums up to 100%
