Try setting up a brand new workflow message. I have found that sometimes when you add fields to an existing workflow message they don't appear.
Any other suggestions?
Yes, both things are set. Email for workflow is enabled and enable email actions is enable also with the correct server name.

Two things to ensure
One that you have the Enable email for workflow enabled
Microsoft Dynamics GP > Tools > setup > system > workflow
Fill in the information for the outgoing and SMTP authentication.
Also, make sure that the box is checked for Enable E-mail Actions. (be sure to enter the name of the server where your Dynamics GP web services are installed)
Second thing to check is in the E-mail message setup that the {%Workflow Action List%} is in the body of the email message
