Contacts are not syncing properly to Outlook ( 1 contact has 8 records now)

I am having an issue where the contacts are not syncing correctly to outlook.  I have check the "System Settings" and the "Personal Options", don't appear to be unusual.  Basically set to contact records owned by current user or relationship Type equal to Vendor and status is active.  What other factors could be causing this to happen?
