Cases - Blank subjects in dropdown menu

I have configured some subjects for cases in Dynamics 365 for service as shown in the first screenshot below:

However when I click the subject dropdown on the Case form I see several blank options:

Any ideas how to resolve? I have tried deleting and re-adding. I am a system admin.
