Lookup record using unique value

Hi

I was wondering if this is even possible.

I have a list of contact record and on a custom form I have a lookup to contacts.

Which is pretty straight forward. 

For each contact record, I have an unique value. 

Is there a way, I can find a contact in the lookup by typing the unique value rather than the contact name.

As with most people, we have duplicate names and wanted to avoid choosing the wrong contact.

Would it also be possible to display the unique value rather than the contact name too.

Look forward to your suggestion or even a work around.

    You can modify the filtering of the fields to achieve it:
