CRM attempts to re-create activitypointer records for tracked emails daily

We recently upgraded to CRM 2015 company-wide.  We are using CRM 2013 for Outlook for email tracking purposes.  

We have had some complaints of slowness so began doing some database traces.  One thing we found were multiple error messages like: Violation of PRIMARY KEY constraint 'ndx_PrimaryKey_ActivityPointer'. Cannot insert duplicate key in object 'dbo.ActivityPointerBase'. The duplicate key value is (f45bc802-7da6-e611-81ae-005056a512df).

Upon further review, we discovered that these messages are being generated multiple times a day (34 times or more for 1 email) for the same tracked emails, day after day (though not for ALL tracked emails) .  

For instance, an email that was created in 7/2016 by Mary Hart will show up again every day, even when the email has not been touched in weeks, and may result in 30+ of these messages each day.  It appears that once they log on to the network, these messages start occurring, so perhaps there is something client side? 

Has anyone else seen this or have any ideas what might be tried to fix it?
