We have a course management system where we want to send surveys to the participants after a course have been completed.
Is it possible to get the responses of the survey to be linked to a specific course (custom entity)? or do we need to create one survey per course to keep the responses separated?

I still dont really get it.
We want to achieve is this:
1. We run a course with our course management system in CRM.
2. When the course is completed we want to send a suvey to all the participants in the coruse. The partipicants is Contacts and is related to the course via a course reservation.
3. The survey should have exactly the same questions for every course, but we want to relate the survey response to a specific course, so that we can do analysis per course.
Survey question might be different according to the course.

It would be better to create different survey for each course rather creating custom entity for each course.
