Company Name Added To Contact - CRM 2016

Hoping someone with more familiarity with CRM can chime in. After updating my exchange PW I was still receiving occasional lockouts from Outlook. I created a new PST file and caught our CRM shared contacts in Outlook. The problem is nearly every contact field now contains the company name. For example what was John Smith is now John Smith - Company Name. I have had this happen in the past as well. When I open the record in Outlook I click the "Full Name" button and there is not sign of the company name. When I click OK after reviewing the full name the contact returns to how it should be displayed John Smith. I do not feel like opening 1981 contact records and clicking "Full Name" followed by OK :/ What is everyone thinking?

If you delete the outlook profile you have on that machine or any other machine. and unconfigure outlook and reconfigure it. Do you see it fixed?
