Survey name not displaying on Survey List

Hi,

When i copy a Survey to use the Questions into a new Survey I get the following issues:

1- The copied Survey name is not displayed on the Survey List and the icon turns orange, see below snap.  If i click on the empty name, the Survey is working fine.  I have cleared cache, got same problem.

Can anyone help!!
