Error in Contact form

hi , i m new in crm , and i work with crm version 2015 , i have a problem in  contact form and i couldn't do any change in this form,  

when i change this form and i want save this error occure ,  object dosen't  support property or method 'toLocaleString' and when i download the error have this message

i cant even save as contact form and continue with new form , could you please help me ?

    Hi,

    The saved query visualization (system chart) entity represents a chart that is owned by an Organization. The schema name for this entity is SavedQueryVisualization. Looks like that is missting for contact form. I will request you create new form, either restore back if you have.

    Hi somayeh.zabihi,

    Welcome to Dynamics.Did you delete any view? You might have deleted a view and some how it might being referenced in the form.

    If not, please try removing all the subgrids in the form ,save and publish the form.Then check if you are facing any issue.If the issue is fixed,then add the subgrids again.
