I worked on a solution a while ago, before UCI was introduced.
The solution stores its Environment settings under (Legacy) Settings and uses the classic interface to display the forms for these settings.
I was recently made aware that the lookup on sub-grids looked strange, which I confirmed locally, on multiple environments and different browsers.
I have also created a new test solution on a fresh environment without other customizations, for which it is also repro.
If I click on the + sign (“Add {0} record.”) on a sub-grid, then the lookup field is placed at a somewhat random location on the page, ex.:
I updated the environment, but the issue persists.
Is this a known issue, and is there a quick fix that can be applied until the settings have been transitioned to UCI?

This is very sad information
I found a total of 7 standard Microsoft forms where this issue was repro and created a ticket at Microsoft with this information along with the initial issue on custom forms, but unfortunately, the response was as expected.
Legacy Settings should be seen as deprecated and will not be supported.

Yes, I have tried to fiddle a bit with the form settings and change the control used by the sub-grid, but nothing really makes a change.
The other lookups (Ex. Field lookup on form and in sub-grid) seem to work fine, it’s just “Add {0} record.” which displays this strange behavior.
I’m seeing the same behavior on standard Microsoft forms, but the number of forms using this logic is quite limited.
One such form is \Settings\Service Management\Queues, select or create a queue and then click on “Add user record” or “Add Record creation and Update Rule record”.

Have you configured the section proprety ?
Try to configure the section dimensions. Else, I think that it's important to create a support request Microsoft.
