Real time plugin is basically used when you want to perform any action in synchronously which means the action should not take more than 2 minutes time otherwise you will get error. So be careful to make a plugin synchronous or real time when you are not sure about the time to take any action.
I am giving you one scenario lets say when creating customer you want to send customer information to another integrated application through WCF service call, in that case I wold always prefer not to use real time plugin because you are not sure about the network and the response time from your integrated system. The scenario is also applicable for update and delete as well.
So I would recommend use real time plugin let say you want to update inside CRM entity or update any field which can quickly executed and you need some value using auto number .

Hi Varun, Real-time Plugin involves, interfacing with webservices to integrate with external systems. for an instance you can try something like when a record created in a custom entity called Feed, post it to Facebook by consuming their API, similar can be done to twitter and linked in. This should give you some hands on.
