I'm facing one wired situation in the Client CRM. The version they are using is CRM 4.0
The problem is the workflow with the name "Update FC Code". This workflow will be run when any record is created in Account entity, but this workflow made as draft on 3-March-2013.
But still now when any record is created in account entity this workflow is triggered and running in the background. When i check this in system jobs, It shows as
Created on : 7-Jun-2014
Started on : 26-Jan-2017.
This is really a challenging situation for me to identify the root cause. Please help me regarding this.

Check system jobs...i think you must have pending system jobs and it may be the root cause of your issue.
Have you tried Resetting your IIS.
The workflow is in draft state
Please check if the Workflow is getting fired from Javascript.
