Hi, I am wondering if dynamics 365 sending meeting request once appointment created, assuming that outlook sync is fine.

Outlook sync and server-side sync are two different things. When you setup your Outlook plugin for the first time you have to enter your username and password. From the sounds of it, your organization is not using server side sync.
To test, create an email in CRM with yourself marked as a recipient. If the status stays as "Pending Send" you are not setup for server side.
If you are using server side sync, it will automatically send.
