I am using Dynamics 365 Online, and something changed about 30-60 days ago and my whole custom form has changed. Many fields have dropped out, including my all important field that contains all my notes. I've gone through each and every unused field, but cannot find this field that contains all my notes. Any idea what's going on here?

Assuming someone hasn't performed a restore from an older copy of your environment, the fields SHOULD still be in the database. To confirm, go to the Default Solution (Settings-->Customizations-->Customize the System) and open the Entity in question. Navigate to Fields and then look for your missing fields on the right. If you find them, then you can simply add them back to the form and, with it, the data should return.
