Orphan Record in Custom Table

Hello All,

I have a custom table and list page I created. For some reason, a blank record keeps getting added to the table. I have tried deleting it, but it keeps reappearing when I navigate back to the list.

Has anyone run into this behavior before?

it needs more code analysis \ tracing to check if there is a trigger inserting this record into table every time when call this page or table.

I found my mistake. I had my cue card flow fields in the table. Those were calculating and inserting a record everytime the role center with the cues loaded.
