When I order NAV 2018 licenses on subscription, running on Azure, does this include SQL Azure as well? Or do I still need to license SQL for each NAV User?

SQL Azure is not licensed with NAV. If you plan to use Azure SQL, you need to activate an Azure Subscription and create an Azure SQL database instance. You pay for the instance, no need to pay CAL (license for users).
