We have some custom case forms in dynamics 365 in a model driven app. Up until yesterday these forms could be selected from the form selector under the case title. This seems to have suddenly stopped showing the forms available in the app.
Any ideas what could be the reason for this.
We have some java script that changes the form to the correct one depending on the subject chosen. This was all working fine.

We have observed the same behavior... initially in our application but have also replicated in standard out of the box apps as well as MS sample apps where the table has multiple forms.
As of yesterday we have only observed it in our Australian environments... but imagine it will roll through to our US environments.
We raised a support ticket yesterday and hope it is resolved soon.
Sam - any update on your problem? - We have seen a few instances of this.
We raised a call with Microsoft. It seemed that making a change and publishing, fixes it but only temporarily. It is happening in all our environments and not just on the case entity.
We also noticed in solution history there were some Microsoft updates over the past few days.
Microsoft haven't been able to fix it yet.

We've had no update yet in getting this fixed
Microsoft support found the issue causing this for us today being in an internal feature called FCB.FormGCMCacheResponse and has applied a fix internally to one environment. We are asking them to find a fix more generally for multiple environments.
