Why when used Move Field for a Form do the columns not adjust in SmartList

Using GP2010 - I created a Form, then used the Move Field to modify the form. I have logged in and out several times, ran Checklinks, and Refreshed the Cache. When I look at the columns in SmartList the Display Column and the Original Name are not in synch. What do I need to do to correct this issue? Any advise would be appreciated. Thanks

I created a Form using Extender. Sorry I did not include that in my original post.
