Error after Submission of the Web Form - success in D365 but error after clicking on submit button

Hello
I have tried everything but currently I am desperate. When we submit a web form without Redirect URL sometimes the web form throws error with error notification message - but the lead is created successfully in the CRM. Sometimes it shows Success notification message and again record is created successfully in the CRM. Not sure if that is valid for MS ticket, but I have not been able to find the reason why this is happening. All required fields are present etc.
