I do have workflow in CRM 2016 it was working fine long time certainly
its giving error.
A record with these values already exists. A duplicate record cannot be created. Select one or more unique values and try again. |

i have seen this error in 2016 alot - whilst i dont think people found answer to this as to why this happens & it might go away in sometime , i would suggest you to try below option :
1. if it is not production do an IIS reset( or in out of hours office , you can also restart Async service)
2. do a deployment including the workflow.
