Support Debugging Tool - Installation Error

We were on an old version of the Support Debugging Tool, version: 11.00.0013

I'm trying to upgrade to SDT Build 16, however, when I install it and launch GP, I am prompted to install the code, sign in, and then I start getting the following errors:

Key Segment Not Found

Error registering tble MBS_ScreenShot_Forms

Unhandled script exception:
Illegal address for field 'Product ID' in script 'MBS_ScreenShot_Save_Form'. Script terminated.

Unfortunately, clicking through these errors just causes them to recycle over and over.

The GP version is GP 2010 - SP2

Any ideas?

    Found the issue with this. In the dex.ini the Dictionary Location was set to a network share. After changing this to the local data path and re-adding the cnk file, it loaded correctly.
