Hi everyone,
I want to upgrade my on CRM 2011 on-premise version to microsoft Dynamics 365 crm on-premise,
I run upgrade custom code from CRM 2011 to CRM 2013 to CRM 2015 to CRM 2016 to Dynamics 365 by using a new instance of SQL Server.
How to migrate data from CRM 2011 to Dynamics 365 on production enviroment after upgrade custom code has finished?

There are different paths available to upgrade your existing CRM 2011 environment to Dynamics 365, You can refer this msdn KB technet.microsoft.com/.../hh699669.aspx once you are on CRM 2016, you can install server updates to get Dynamics 365
You should be able to take complete back of your organization and restore it in the new SQL Server instance, otherwise you can use tools SSIS toolkit, scribe etc

You can migrate data from CRM 2011 to Dynamics 365 using SQL Server Integration Services (SSIS) and the KingswaySoft SSIS Integration Toolkit for Microsoft Dynamics 365.
