The email from name needs to be set up in the summary tab of the email you are wishing to test before you try to test send the email. Also, the email from name does not need to be a licensed user.

Hi, are you using personalization into your email for the from name or is it a static value? if the latter it should show up in test send, so you might want to open a SR for us to look into it.
For the second question. No, the "email from address" can be any email address. for deliverability reasons we suggest to configure your sending domain so that emails will have a better deliverability to your recipients, obviously.
