I've gone over the instructions on putting notes on entity forms in detail and it's just not working. The Entity Form is for Incidents in edit mode. The CRM form has Notes on it. The Entity Form Metadata is setup exactly as the example. The Entity Permissions are set up for everything except delete for Authenticated Portal Users. I include the links above so nobody will just suggest I read those links. :-)
Anyone out there have similar issues? Any ideas where to look next? We are using D365 On-Premises.

PS: The "Filters" below is set to Don't Know because there's no option for D365 On-Premises.
