Hi all,

Path: Personnel Management/ (Employees/ Contractors)- Worker Tab- Checklists (On Boarding/ Off Boarding and Transition)

I need to add a couple of  fields in the checklist  Approved date and approved by

how to get the approval details of checklist 

how to find where this information is getting stored

Kindly help me

    Hi Hasini,

    If you go to form > Options > Personalize this page  > Add a field and select field available from List.

    then Do you see the fields needed for you?

    If NO then You have to use a custom field option
