Lead Scoring Model - Survey/Customer Voice entity

Hello guys
There is a scenario where initially the customer journey is runned for lead generation for those who fill out the survey. The lead is generated but this based on the survey response I would like to add that in the lead scoring and assign points. The following are the two issue:
1) Lead is generated but there is activity of that mentioned in timeline that the customer response has been sent and received.
2) Following that in the lead scoring model I cannot find the entity of quotes, customer voice/survey also.
 
Kindly help out as this is urgent.
