Form Field Failure - Failed to convert value (Multiple values?)

The form we've created using a checkbox field is failing with either single or multiple entries made for the checkbox field. 
Can anyone advise on how to remedy this error?

    Hi JSADev,
     
    Based on the screenshot you provided, the problem is that the values passed in by the marketing form are required to be converted to virtual.
    However, the various limitations of virtual entities are unlikely to allow this behavior, so the creation fails:
     
    The contact form is managed, so we can't set it to a virtual entity.
    I'm guessing that having the contact table connected to an external data source in your environment, or a customization that stores data captured by marketing forms in an external database triggers these issues.
     
    Also, checkbox fields only support boolean types, which obviously don't match the three fields in your example, so it is recommended that you try option sets or other appropriate field types.
    There are other caveats in the official documentation (e.g., do not map multiple marketing form fields to the same field in the database):
