Creating Online Form with action buttons

I am looking to find a way to create a form that is actually an internal application for candidates to fill out when they want to be granted a certain role in our company. In an effort to automate this in the past we created the application in Survey Monkey. However, the process for applying for the role requires that they fill out and submit the application which is currently in Survey Monkey and then they have to remember to go out and email a copy of their resume to me.

What I would like to do is find a method that would allow the following steps:

1. Fill out the application in a form

2. Have a way for a user to attach their resume to the form

3. Provide submit button that can only be used after the resume is submitted

4. Have the completed resume route to an approver via email where the approve is prompted to click approve once they review the information

5. Be able to go back into the system and see a list of everyone that has submitted an application

Does anyone know if we can do this with any of he 365 applications? Thanks.

Hi Chelle Young,

You can satisfy your requirments by creating a canevas Power apps and a Power automate flow

Step 1 : Create a Canevas Power apps from make.powerapps.com

In this app, you can add a control that contain the fields that you want, where the user can fill thsese fields

You add also the attach file control to the form that permit users to attach their resumes

Step 2 : Create a Power automate Flow with the Power apps trigger and a conddition control to the flow to check if the user has attached their resume,

Add an "Office 365 Outlook - Send an email" action to the flow to send an email to the approver

Add an approval step in your flow

Step 3 : You can add a gallery in your canevas app to display the list of users that send their resumes.
