Unfortunately you did not mention what you already know and what you have already tried to do.
In order to modify a dataport's request form, design the dataport object using the object designer.
Under View --> Request Form, you are able to add controls. However, you will have to consider this: http://www.mibuso.com/howtoinfo.asp?FileID=4
