You can reach the menu I shared in the picture by right-clicking on the form.

You may have hide it
There is another possibility, the value is empty. It comes from the text of the headerTitle. Either the headerTitle is empty, or it have been overrided in the code with "headerTitle.text()" and overrided value is empty
Finally, they may have removed the field from the form or made the visible no.

I would suggested to delete usage data at first and try the for again. Do you face the issue only on one form ? Or others as well ?
