Format a Field for Phone Numbers

We have try all different ways of java script, but I am unable to get it to work so, our phone field in our web form on out portal site to auto format. When a customer is type out the phone number it would show something like this example (123)456-7891 in the field.

We are running Dynamic 365 Version 1710 (9.1.0.2450) online.

PLEASE HELP, IF anyone could help me with this process. All I could find was on the older version of Dynamics and for some reason I can not get it to work or doing something wrong.
