    Debi,

    IRS does not want you to report SSN/TIN, and DOB, for a covered individual. You should only be reporting DOB if SSN/TIN is not available.

    Is the SSN/TIN showing up correctly? If so, that's likely the cause.

    The text on the form reads "(c) DOB (If SSN or other TIN is not available)"
