Show questionnaires? = Yes but questionnaire never appears on purchase requisition line

What we are trying to achieve

We buy software licenses through purchase requisitions. The requester chooses a procurement category and we need the requester to record a few structured values per requisition line at the moment they raise the request:

- license start date
- license end date
- whether the license auto-renews
- notice period in days before non-renewal

What we configured

Questionnaire module (Questionnaire > Setup / Design)

1. Question type — the Question types table was empty, so we created one.
2. Questionnaire type — same, the Questionnaire types table was empty, so we created one.
3. Answer groups — one Yes/No group and one multiple-choice group, each with answers.
4. Questions — five questions: two with input type Date, one Integer, two Combo box linked to the answer groups.
5. Questionnaire — created, all five questions attached in sequence, Active = Yes. Validate returns "No errors detected".

Procurement category (Procurement and sourcing > Procurement categories)

6. On the software category, Questionnaires FastTab > Add > selected our questionnaire, and set Activity type = Requisition.  "Inherit questionnaires from parent category" = No.

Purchasing policy (Procurement and sourcing > Setup > Policies > Purchasing policies)

7. In the policy whose Policy organizations match the requester's operating unit, Policy rules > Category policy rule > added the software category to Category policy data and ticked Show questionnaires? = Yes. The rule is currently effective (effective date in the past, expiration 31/12/2154).

What happens

Nothing. We create a new purchase requisition, add a line, select the software category, complete the line so that it saves (quantity, unit, unit price — line number is assigned, so the line is committed).

- No questionnaire dialog appears when the category is selected.
- Line details > Questionnaire is empty.
- Complete questionnaire and View answers are both greyed out.

There is no error, no warning and no message.

Simplified reproduction

To rule out anything specific to our five-question design, we built a second questionnaire with a single question of input type Text, activated it, attached it to the same category with Activity type = Requisition, and created another new requisition line. Identical result — nothing appears.

    Hi,

    What is the “Vendor selectin” option selected in the category policy rule? Try selecting “Allow all vendors” and Show questionnares= Yes.

    I tested and I am able to view the questionnaries in the PU lines.
